What is a Medicare Annual Wellness Visit?

 
The Annual Wellness Visit (AWV) is a special visit created by the Centers for Medicare & Medicaid Services to help ensure the health and well-being of Medicare beneficiaries. This visit is designed to focus on preventive care, keeping you in the best possible health. Your AWV takes place once a year, and it involves meeting with your primary care clinician. During this visit, the primary goal is to identify and address any potential health risks you might face. Your clinician will also work with you to create or update a personalized prevention plan that is tailored specifically to your needs and health goals. This visit is covered by Medicare without a share of cost/copay. Please complete the following forms and bring them with you to your appointment.
